Diabetic Foot Surgery

Diabetic Foot Surgery
As Op. Dr. Halil İbrahim Öçalan, we provide expert diabetic foot surgery services in Gaziantep. Diabetic foot refers to serious complications seen in diabetes patients due to nerve damage (neuropathy), circulatory disorders and immune system weakening. With early diagnosis and appropriate treatment amputation risk can be significantly reduced.
What Is Diabetic Foot Syndrome?
Prolonged high blood sugar causes nerve damage (diabetic neuropathy) and narrowing of blood vessels in diabetes. This condition creates loss of sensation in feet, deformities, wound formation and infection risk. If diabetic foot ulcers are not treated gangrene can develop requiring amputation.
Risk Factors
- Long-term diabetes (over 10 years)
- Poor blood sugar control
- Diabetic neuropathy (sensory loss)
- Peripheral arterial disease (circulatory disorder)
- Foot deformities (bunions, clubfoot, hammertoe)
- Previous foot injuries or amputations
- Smoking
- Kidney disease
Diabetic Foot Ulcers
Sensory neuropathy reduces pain sensation in feet so small wounds go unnoticed and grow. Ulcers formed at pressure points become prone to infection.
Ulcer Classification (Wagner)
- Level 0: Skin integrity preserved, risk area
- Level 1: Superficial ulcer
- Level 2: Deep ulcer, no bone or joint involvement
- Level 3: Deep ulcer with osteomyelitis (bone infection)
- Level 4: Localized gangrene (heel or toes)
- Level 5: Generalized gangrene
Treatment Approaches
Diabetic foot treatment requires multidisciplinary approach. Our goal is to promote wound healing, control infection and prevent amputation.
Non-Surgical Treatment
- Blood sugar optimization (endocrinology collaboration)
- Circulation assessment and vascular surgery consultation if needed
- Wound care and debridement (dead tissue removal)
- Pain management
- Compression therapy (edema control)
- Offloading devices (pressure reduction)
Surgical Treatment
- Debridement: Surgical cleaning of dead and infected tissue
- Revascularization: Bypass or angioplasty in vessel blockages
- Osteomyelitis treatment: Bone resection in bone infection
- Deformity correction: Osteotomy to eliminate pressure points
- Burow surgery: Emergency debridement in severe infections
Amputation Prevention
In diabetic foot amputation is last resort. With early intervention many patients can preserve their limb. Multidisciplinary foot clinics play important role.
Prevention Strategies
- Daily foot examination (self or family member)
- Washing and drying feet daily
- Moisturizing cream use (except between toes)
- Regular shoe inspection
- Protective socks
- Avoiding risky activities (hot water, barefoot near heaters)
- Professional foot care (pedicure by podiatrist)
